New Patients

We advise all new patients to arrive for their new appointment approximately 15-20 minutes ahead of their scheduled appointment time. This will allow ample time to fill out necessary paperwork. In order to expedite this first appointment a health history can be downloaded here and can be filled out before you arrive for your appointment.
